Who It’s For

Hiring teams tracking required previous-employer safety inquiries.

How to Use It

Use one sheet per previous DOT-regulated employer. Keep the inquiry, response, and supporting correspondence in the restricted driver investigation history file. This log tracks the process; it does not replace the inquiry, required authorizations, driver notices, or returned information.

What’s Included

  • Employer and request details
  • Contact-attempt and response log
  • Thirty-day review and missing-record follow-up

Under 391.23, required responses or documentation of good-faith efforts belong in the driver investigation history file within 30 days after employment begins. The rule does not make a fixed number of unanswered calls automatically sufficient.

Apply the current Clearinghouse and other-DOT-employer rules when determining how prior drug-and-alcohol history is obtained. Do not place test results or medical details on this general tracking sheet.
