Who It’s For

Safety administrators and owner-operators monitoring driver qualification dates.

How to Use It

Create one sheet per driver. Enter dates from current authoritative records, not memory. Set reminders early enough to complete the renewal and verify the new status. Keep copies and medical records in the appropriate controlled files.

What’s Included

  • Credential and review schedule
  • Reminder owner and completion evidence
  • Restrictions and assignment follow-up

Use actual expiration dates and applicable review intervals. A medical certificate is not automatically an annual renewal. For CDL medical status, check the required licensing-state record and applicable current procedures; a reminder does not extend an expired credential.

This tracker does not replace the annual MVR review, required qualification evidence, or a check of current driving eligibility before assigning work. Avoid Social Security numbers and medical diagnoses on this sheet.
